In May of 2008, Community Opportunities and the Department of Mental Health
entered into a partnership to provide local case management. The goals of
this partnership are to:
- Reduce caseload sizes.
- Meet the needs of individuals in a more timely and efficient manner.
- Enhance case management in the county and provide quality services to
individuals and families..
- To provide a more flexible service delivery system.
- To provide a choice to the families of Lincoln County.
Service coordination, as provided under this program, is a group of activities that assist individuals to gain access to the care and services they need. Case Managers are also responsible for monitoring the effectiveness of the services and supports being given. Case Management activities may take place in or out of the presence of the individual being assisted, and may include contacts with others, assessments, planning, monitoring and documenting on behalf of that individual. Community Opportunities Case Management is partially funded through the Department of Mental Health, Division of MR/DD, Medicaid and Lincoln County tax dollars.

Regional Office providing your case management services. This will take
place at your annual person-centered planning meeting or at any time you
desire.
- Response to phone messages and other communications in a timely manner.
- We will listen to what you are saying.
- An opportunity to share your ideas on how we can improve or change our
case management.
- You will have the lead in planning the supports you need.
- To be kept up to date on information that could have an impact on services
you receive.
